This Privacy Notice describes what information we collect about you, how we collect it, how we use it, with whom we may share it, and what choices you have regarding it, through www.TheLabCafe.com or other websites controlled by OTF Health (collectively, the "Site"). By accessing or using the Site, you acknowledge that you have read our privacy practices (and any uses and disclosures of information about you) that are described in this Privacy Notice.

OTF Health is not a "covered entity" or a "business associate" as defined in the Health Insurance Portability and Accountability Act ("HIPAA") and is not subject to regulatory enforcement for non-compliance with HIPAA. OTF Health agrees to make commercially reasonable efforts to comply with the health information management requirements of HIPAA as outlined below and in the Notice of Privacy Practices.

What information do we collect and how?

Information Collected From You

We may collect certain information from or about you in connection with your use of or submissions to the Site, including, but not limited to, your name, address, date of birth, email address, health information (including symptoms, conditions, medications, pregnancy status, and test results), health plan information, gender, age, height, and weight. We also use third-party payment processors to collect payment information like your credit card number. If you use our Site on behalf of someone else (like a child or an elder), we collect similar information about that person. Health information is collected either through the Site (such as when you use our "help choose a lab test" feature) or by authorized providers or testing laboratories who help us provide the services. Our use of such health information is governed by our Notice of Privacy Practices. In addition, we may retain the content of, and metadata regarding, any correspondence you may have with us or our representatives, regardless of the mode of communication by which such correspondence was made.

Information Collected Automatically

As with many other websites, the web servers used to operate the Site may collect certain data about the equipment and communications method that you use to access the Internet and our Site as well as information about how you use our Site. The information we collect may include the Internet protocol ("IP") address assigned to your computer, user agent, referrer, GCLID, specific pages that you accessed on the Site or prior to visiting the Site, and the length of time you spent at the Site.

Cookies, Pixels, and Similar Technologies

Technologies such as cookies, pixels, JavaScript, or similar technologies, are used by OTF Health and our marketing and analytics partners. These technologies are used in analyzing trends, administering the Site, tracking users' movements around the Site and to gather demographic information about our user base as a whole. We may receive reports based on the use of these technologies by our partners on an individual as well as aggregated basis.

If you submit personal information (including any health information) to any of those sites, for example, an authorized partner or testing laboratories, different rules may apply to their use or disclosure of the information you disclose to them. We do not control the privacy policies or practices of third parties, including our partners. You are subject to the privacy policies and practices of those third parties, as applicable.

When you click on links on the Site you may leave our site. We are not responsible for the privacy practices of other sites, and we encourage you to read their privacy statements and/or policies.

Our website may include widgets, such as Google Maps and embedded video players. These features may collect your IP address, which page you are visiting on the Site, and may set a cookie to enable the feature to function properly. Widgets are either hosted by a third party or hosted directly on the Site. Your interactions with these features are governed by the privacy policy of the company providing it.

Analytics Technologies

In addition, the Site may use third-party web analytics and session-replay services. These services may record mouse clicks, mouse movements, scrolling activity, keystrokes, and Site events, such as adding items to your cart, making orders, or requesting doctor consultations, as well as other data inputs, while you navigate through the Site. Furthermore, OTF Health does not track your browsing habits across websites we do not own. How do we use the information you provide us?

With whom do we share the information we collect?

We share your personal information with third parties as described in this Privacy Notice, the Notice of Privacy Practices, or any service agreement you may enter into with OTF Health.

We may share information with other companies and authorized agents that provide services to us, including the providers of analytics and advertising cookies described above. If all or part of OTF Health is sold, merged, or otherwise transferred to another entity, we may transfer information to such entity as part of or in connection with diligence about that transaction. How Do We Protect Information Collected About You?

OTF Health maintains certain commercially reasonable security measures designed to protect information, both during transmission and once we receive it.
